Request for copies of medical records created 2016-2021 can be made to:
A signed “release of records form”, by the patient including, DOB and address when the service occurred is required.
Records are only released to a Medical Practitioner where ongoing care is needed.
No clinical matters can be addressed
Requests for records may take up in excess of 30 days
